Leggett Prosthetics LLC is in-network with the insurance providers listed below
Leggett Prosthetics is ready and willing to assist any patient; even If we are not in-network with their provider. The patient may be responsible for a larger percentage of the cost, subject to the out-of- network rate set by the insurance plan.
Prosthetic Device Coverage
Upon receipt of the patient’s insurance information, our experienced office staff will verify benefits and determine the amount of coverage available for prosthetic services. Our office will obtain prior authorization, if necessary for your plan; and all patients will be given an estimate of out-of-pocket cost prior to services.
Please note: A prescription is required prior to providing clinical care.
(A prescription may be written on the Doctors script pad and be written as simple as “Eval and treat for prosthetic device and/or supplies”)
